**Job Advert Summary**:
**Purpose of the position**:
The purpose of this position is to ensure proper screening, testing, linkage of TB clients to care and support services as well as facilitation of retention to care according to DOH guidelines. The incumbent will also ensure that TB clients are linked to a relevant facility and as well as community services.
**Minimum Requirements**:
- Grade 12
- Higher certificate in auxiliary nursing or an equivalent qualification as nursing auxiliary
- Knowledge of basic TB, and HIV management
- Experience in health promotion and health education
- Current registration with SANC as an auxiliary nurse
- 2 years of working experience within a similar environment or any clinical setting
- Experience in working with outreach teams & providing health education. Community TB screening will be an advantage
- Computer literacy in the Microsoft Office package
- Proficiency in verbal and written communication skills
- Willing to travel and work after hours
- Clear criminal record
**Duties and Responsibilities**:
- Counselling of newly enrolled MDR client
- Identify barriers to treatment adherence
- Contact tracing and referral to facility
- Facility follow up counselling sessions to emphasise adherence
- Recall treatment interrupters
- Refer for psychosocial support to social worker services
- Do a one-on-one session with the client to identify contacts
- Home visit to ensure contacts are traced and referred to clinic
- Under 5 contacts are placed on IPT
- Follow up in client’s record if the referred contacts attended the facility
- Make appointments with MDR clients for counselling sessions
- Build relations with other CBS / NPOs who deliver services in the community
- Community profiling with list of non-government and FBO’s services
- Complete eligibility checklist for each client
- Ensure the client receive the monthly support from allocated NPO
- Ensure the client attend the facility for sputum follow up dates
- Give a monthly report according to set expectations outlined in the SOP
- Perform irregular, one time and special request tasks related to the department as a whole
- Perform activities as stand-in for colleague or supervisor as required from time to time
