**Job Advert Summary**:
To provide Home and community based care programmes incorporating screening of non-communicable diseases, TB, HIV and STI’s including psycho-social needs within the sub-district.

**Minimum Requirements**:

- Certificate in Enrolled Nursing
- 2 years’ experience in the capacity of an enrolled nurse
- HIV counselling and testing in public health services.
- SANC registered as an enrolled nurse for the current year
- Fluent in IsiXhosa and/or English languages
- Must be fully vaccinated against COVID19

**Duties and Responsibilities**:

- Provides mentorship to community health workers
- Coordinates and conducts awareness outreach in the community, workplace, tertiary educational facilities and schools in line with the health calendar.
- Appropriate referral of all cases identified in the community
- Ensures that clients referred to the Home and community based care programme are managed
- Promotes service delivery and identifies training needs
- Networking and maintains a healthy and positive relationship with the sub-district
- Administration
- Adheres to Infection control Standards
