About the position:

The role of a Support Coordinator involves coordinating supports for NDIS Clients and assisting them in achieving their goals outlined in their NDIS funded plan. This requires identifying, developing, coordinating, and facilitating access to supports and services alongside allocated NDIS Clients.

The ideal candidate will be a highly organized individual with excellent oral, written, and interpersonal communication skills. They must be able to work independently and cooperatively as part of a team, demonstrating self-discipline and effective time management.

This role requires analytical and creative skills to analyze individual needs, identify and measure outcomes, and report on progress. The successful candidate will have a positive and contemporary attitude towards people with disabilities and the ability to interact, communicate, and engage effectively with diverse cultures.

Key responsibilities:

  • Collaborate with clients, families, and carers to identify and access opportunities for skills development, education, and employment.
  • Provide support coordination services in line with NDIS plans and encourage clients to achieve their goals.
  • Develop networks and collaborate with stakeholders to proactively identify, plan, implement, and evaluate supports.
  • Prepare and maintain accurate case management notes and reports using clear and concise language on client progress.
  • Liaise with various stakeholders, community services, government agencies, customers, and service providers.
