Intercare Tygervalley Sub Acute Hospital situated in Bellville, Cape Town seeks to recruit Assistant Case Manager. The purpose of the position is to ensure that all patients admitted have the necessary authorization by their medical aid and to assist with case manager with reporting and other ad hoc requirements.

**CRITICAL OUTPUTS**
- Ensure that the patient flow process is smooth, efficient, and conducive to patient satisfaction.
- Liaise with funders and referring parties with regards to transfer of patients.
- Assist patients with authorization where required.
- Communicate with health care professionals where required with regards to patient benefits and limits.
- Obtain admission documentation from patients prior to admission.
- Escalate any areas of concern to appropriate line manager where required.
- Coordinate feedback from multi-disciplinary team and provide referring doctors and funders with detailed management, progress, and discharge reports.
- Review admission schedules on a daily basis and confirm authorization, ICD10 codes with medical aids.
- Assist case manager with administrative duties including authorization, extension of length of stay, distribution of reports and auditing
- Capture admission information in time, correctly and update as required.
- Capture referring and attending doctors on the system in line with the process.
- Communicate all payments required to patients and collect prior and during the episode of admissions.
- Refer complex account queries to line manager.
- Ensure 100% accuracy in taking and allocating of payments.
- Identify patients and manage total patient case from admission to discharge between funders and hospital.
- Coordinate and update all patient information, such as authorizations etc. between admin staff and funders.
- Build and maintain productive working relationships with all key stakeholders through on-going communication and feedback.
- Proactive communication regarding changes and initiatives to key stakeholders.
- Identify, monitor, resolve, communicate and where necessary escalate deviations that may lead to discontentment and decreased occupancy levels.
- Be aware of budgetary targets with regards to occupancy levels and work as part of the team to achieve set targets.

**REQUIREMENTS**
- Minimum Grade 12
- Nursing qualification would be advantageous
- Working knowledge of ICD & CPT coding required
- Administration experience in a medical facility
- Understanding of medical schemes and admission processes required
- Knowledge of the AS400 system will be advantageous
- Computer literacy on all MS Office packages
