We are looking to fill a vacancy of Youth Transitions Facilitator in our Youth Development Programme, which focuses specifically on young people 16-23 who have grown up in Child and Youth Care Centres.
Key responsibilities include- To offer transitional support to young people aged 16 23, who have grown up in Alternative Care (Child and Youth Care Centres (CYCC's) and Foster Care).
- To provide ongoing individual oneonone mentoring and support to young people in preparation for their transition and after they leave care.
- To plan facilitate monthly experiential group sessions, wilderness processes and camps.
- To facilitate practical support, assisting young people to access further education, accommodation, skills training and job placement as part of supporting a successful transition from Alternative Care.
