  • Qualification: Minimum Certificate optional Diploma in Project Management
  • Previous experience in Health Care, NGO Setting would be beneficial.
  • Proficient in Ms. office Suite
  • Excellent Verbal and Written Communication Skills

Must have:

  • Driver's License a must for this position as there is travelling involved
    .
  • Clear police record.

  • Knowledge of relevant legislation and regulations.

Job Scope

  • Present reports at district meetings with the Department of Health.
  • Interact with Sub District coordinators when requested.
  • Guide CHWs and OTLs.
  • Work with the OTLs/network partners to design the yearly health calendar.
  • Examine referrals for COPC and determine whether to accept or reject them.
  • Refer to network partners if the referral is denied.
  • Assign patients to areas/professional staff.
  • Ensure each patient has an appropriate care plan.
  • Facilitate weekly IDT meetings.
  • Execute a 6 monthly audit of 10% of each area's records.
  • Build positive relationships with OTLs and CHWs to provide ongoing informal support.
  • Monitor the number of patient deaths of each nurse and be mindful of the risk of emotional burnout.
  • Train and educate OTLs and CHWs.
  • Assist OTLs in setting up professional boundaries with patients and families.
  • Have informal conversations with any OTLs or CHWs that have been on the team for less than 1 year to assess how they are fitting in.
  • Examine the OTLs monthly reports the activities should match the business plans. submitted to the Department of Health. Analyses and submits to Chief Operations Manager on a monthly and quarterly basis.
  • Monitor OTLs attendance registers.
  • Supervise medication for home care deliveries.
  • Monitor patient loads per work area.
  • Refer team members to external professionals for support when necessary.
  • Lead formal meetings with each subdistrict at least once a week.
  • Evaluate the professional levels of service provided by the team members.
  • Monitor staff for compassion fatigue.
  • Organize group or individual debriefing as required.
  • Discuss training opportunities with staff and enable them to attend.
  • Lead performance management system for staff, including ensuring that an annual performance appraisal is completed.
  • Monitor patient loads per work area.
  • Responsible for Personnel Development of staff
  • Management of all reports and ensure that all administrative functions are carried out.
