At Livewell, we pride ourselves on providing specialised Dementia and Alzheimer's care services, to Residents.

We are looking to hire Care Providers with experience in Geriatric Patients diagnosed with Dementia.

Responsibilities include, but are not limited to:

Daily Care & Wellbeing of Livewell residents

  • Giving personal care of high standard to Livewell residents assigned to you and the following:
  • Personal hygiene
  • Oral hygiene
  • Washing/combing/grooming of hair, ensuring hair is clean and neat
  • Washing (bath/shower) regularly
  • Assisting with dressing of clean and appropriate clothes according to season
  • Shaving of beard
  • Grooming nose and ear hair
  • Reporting of grooming needs to the Quality of Life Leader
  • 2 to 3 hourly pressure care when needed
  • Assisting with toilet and/or incontinence wear
  • Reporting of toiletries needed by Residents, to Shift Leader
  • Following resident kit list and complying with laundry procedures
  • Reporting of any torn clothing of residents assigned to you to the Quality of Life Leader
  • Monitoring residents as instructed on care plan

Mealtimes & nutrition (Assisting the residents during mealtimes, understanding nutritional needs and fluids

  • Assisting residents with feeding and use assistive devices as per OT requests
  • Prevention of choking by handling food
  • Protecting residents' clothes in a polite way (napkins)
  • Reporting lack of appetite or signs of discomfort or dehydration and attending to the thorough completion of the intake and output chart

Ensure the safety of the residents

  • Complying with safety measures and regulations to prevent injuries to residents and themselves.
- fall risks
- fire risks
- reporting elderly abuse (emotional, physical, financial) according to the Older Person's Act

  • Residents should be within the Livewell premises at all times and closely monitored when on an outing
  • Regular monitoring (at least every half an hour) of all allocated residents to ensure their safety, comfort and wellbeing

Creating and maintaining a clean, tidy environment for Livewell residents

  • Making residents' beds and ensure that the linen is clean. Soiled linen should be taken to the laundry
  • Keeping resident's room neat and tidy
  • Reporting operational issues
  • Cleaning of resident's cupboards
  • Taking out laundry and filling in the laundry booklet (checking out and in of laundry)
  • Disposing of incontinence wear properly and adhering to infection control policy

Recordkeeping, Reports and Administration

  • Recording all care and assistance rendered
  • Completing all daily care documents, ensuring these reports are up to date for the residents assigned to you, including but not limited to:
  • in-and output documents, as and when required or requested
- reporting of bruises or physical changes to your Shift Leader, and completing the Holistic Resident Care Checklist & Report, as well as Incident Report
- completing ABC chart in case of behaviour changes
- reporting any emotional or awareness changes, or abnormalities to the Shift Leader immediately, and completing such checks on the ABC Chart and Holistic Resident Care Checklist & Report
- ensuring hand-over report is accurate and completed on time and sign-off by the Nursing Shift Leader prior to end of shift

  • Making concise, accurate and wellorganized clinical notes and verbal reporting on the wellbeing of my residents, while following the correct lines of communication

Assisting residents in activities of daily living to promote their well-being

  • Assisting Quality of Life Leaders, companions and therapists in exercises and activities
  • Getting the residents ready for an activity according to the activity program timeously
  • Continuing with activities when there is no Quality of Life Leader, companion or therapist available
  • Ensuring that residents are stimulated, attentive to their needs and keeping up to date with the activity program to assist with optimal attendance of residents assigned to you
  • Using electronic devices like radios and televisions to keep residents occupied, but using applicable channels
  • Reporting on resident's interaction and/or lack of interaction or interest in an activity, to contribute to provide each resident with optimal quality of life

Ensuring that infection control is maintained on the floor

  • Complying with the Infection Control measures as per the Infection Control Policies and Procedures of the company
  • Attending to regular washing of residents' hands, especially before and after meals, and bathroom routines
  • Adhering to good personal hygiene
  • Incontinence wear should be removed to allocated areas as soon as possible
  • Washing of hands should be done before and after resident care

  • Maintaining and having a positive attitude towards residents, staff, families and the company to contribute to a good working environment, be selfmotivated and willing and keen to lear
