Faircape Health is a leading private healthcare provider in the Western Cape with over 300 beds, offering on-site Healthcare Centres at all of our Faircape Life retirement villages as well as a Health Rehabilitation Centre at Tokai Estate.

Our extensively trained staff and comprehensive activity programmes enable the provision of frail, dementia, palliative care, restorative care and rehabilitation to patients in a safe and secure environment.

We are seeking a **Social Worker **to provide support and services to residents within the healthcares and villages as required. The Social Worker will focus mainly on three of our facilities namely Tokai Estate, Cle Du Cap and Noordhoek Manor (Southern Suburbs) to do psychosocial assessments,advocate for the residents’ best interest and to provide emotional support.

**Duties will include the following, but not limited to**:

- Support and counselling to residents and/or family members
- Assistance with adjustment to admission or change in health status
- Assistance with care decision making in absence of family
- Assistance with managing difficult family situations
- Implementation of the Older Persons Act 13 of 2006
- Present therapeutic group sessions to residents to improve quality of life and foster positive relationships.
- Participating in monthly MDT meetings
- Providing training to various members of staff based on need and request
- Writing health educational/promotional material for newsletters to families and residents

**Requirements**:
**Qualification**:

- Bachelor of Social Work

**Experience & Knowledge**:

- 2 years experience in a similar role
- Training and experience in counselling
- Experience in statutory processes
- Training in bereavement, palliative care and dementia care will be beneficial
- Knowledge of the Older Person Act advantageous

**Skills & Attributes**:

- Computer literate
- Excellent communication skills (verbal and written)
- Excellent organisational and time management skills
- Able to perform under pressure
- Deadline driven
- Demonstrate respect for the individuality of the patients and families
- Ability to establish and maintain collaborative and effective working relationships
- Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills

**Specific Requirements**:

- Registered with SACSSP
- No criminal record
- Drivers licence and own transport
- Vaccinated against covid-19 or prepared to be vaccinated prior to commencing job

Normal working hours are Mondays to Fridays from 08h00 until 17h00 with a 45 minute lunch break. The company allows employees, by approval of the direct manager, to work flexi time between 07h00 and 18h00.

Faircape offers a market-related salary from R24 000 to R30 000 Cost to Company per month, depending on experience.
