Location: Johannesburg- Position Type: Permanent**Purpose of the Job**:
**_To fulfill the position of first and only point of entry for all Jewish community members to the Chevrah Kadisha, in a compassionate, kind, caring and efficient manner._**
**_ To provide an opportunity for community members to request assistance, be understood and contained through the promotion of service options and resources by facilitating psychosocial, financial, admission and home-based care assessments. _**
**_Short term engagement with callers, persons and /or families based on assessment and referral for service delivery within and external to the Chevrah Kadisha dependent on the need._**
**_ To uphold confidentiality, privacy and privileged information in a dignified and respectful manner, within the confines of the Group, POPI Act, relevant Registration Councils with and without consent of the individual as may be determined_**

**Role and Responsibilities**:
**_Containment_**
- To listen and understand the caller or persons request, need or challenge.
- Immediate crisis management and referral to Chev Community Social Services or financial care assessment services for action or follow through as required
- To explain and guide client through the process of admission / service provision and explain what documentation will be required of the client and time frames of processes and service delivery
- To facilitate admissions timeously
- To provide support, care and compassion
- Verification of Jewishness
- Terminate and/ or refer
- Provide emergency dry stock, toiletries, petty cash
- Ensure immediate safety and protection (safeguarding)
- Follow up as required
**_ Assessment: _**
- To assess all referrals to the Chevrah Kadisha requiring the organizations service offerings.
- Conduct face to face office interviews (walk-ins or scheduled), or online assessments. These assessments can be telephonic if deemed appropriate. Collate all required documentation.
- Home visits
- Hospital visits
- Trauma containment
- Assess, report and refer abuse and at-risk matters with immediacy to the Chev Community Social Services Family Protection Unit
- To work together with significant others, professionals, other organization, Chevrah departments and divisions, communal organizations and rabbi as referral sources
- To assess, process and refer financial enquiries for financial assistance. Collate all required documentation.
- Ascertain whether the client seeks a subsidy (for admission) and explain subsidy process and requirements.
- Ascertain whether the client seeks a full fee admission and explain process and requirements.
- To provide recommendations and suitable services options and refer accordingly
- To communicate effectively and efficiently both oral and written
- To provide support, care and compassion
- To develop ongoing resource lists for referral and specialist services delivery
- Follow up, short term monitoring and liaising as required
- Terminate and / or refer
**_ _**_Referral:_**
- After assessments, refer to Chevrah departments, entities or community resources:
- Assessment for admission to the suggested facility
- Social services
- Emergency admissions
- Crisis intervention
- Other affiliated community organizations
- Other NGO’s
- Financial assessments
- Rabbinical support
- Pro bono legal services
- Medical, psychiatric and rehabilitation services or institutions
- Other
- Receive, check and collate documentation from client and forward relevant information to Assessment team when received.
- Once the person/ family been assessed for any of the Chev services and resources, make recommendations, process with the person/ family and allocate and refer accordingly.
- Follow up, short term monitoring and liaising may be required
- Terminate and refer
**Administration**:
- Capture all client information electronically
- Complete assessment reports, request and obtain required supplementary documentation as part of the assessment process timeously
- Complete intake packs and contracts together with concerned persons as required
- Keep updated process notes/ records on the process until termination and referral
- Ensure all relevant documentation and reports for ongoing assessments by other Chev entities or external organizations provided
- Update Chevnet for new applicants, which could include address book and pre-admissions
- Maintain statistics
- Writing relevant reports as appropriate
- Develop and update resource lists
- Have access to Chevnet and relevant databases to capture and verify information
