Responsibilities

The Hep C Program Coordinator is responsible for the coordination of activities relating to the delivery of services for the Hepatitis C infected population. The coordinator supervises all program related activities including outreach/in-reach collaborations, patient engagement & enrollment, care & treatment delivery, and grant reporting.

Work with staff from HOPE Center and designated consultants and/or contractors to design, implement and coordinate an evidence-based Hepatitis C readiness and treatment program. Recruit Hep C positive patients for Hepatitis C treatment.
Assess and monitor the mental health and medical readiness of patients to initiate Hepatitis C treatment through the implementation of Prep-C and other program related assessments and screens. Coordinates and directs patient care activities, delegates tasks appropriately and accepts the accountability of her/his professional role.
Document required core data elements of care in the AIRS system for federal and state reporting and participation in quality improvement projects.


Collaborates with other health care disciplines, including staff from the New York State’s Department of Health, AIDS Institute to coordinate and implement patient care activities and discharge plans.
Keep current on all issues relating to the treatment of Hepatitis C and keep abreast of professional literature pertaining to Hepatitis C. Information is utilized in the delivery of care, coordination, and provision of training to staff at the HOPE Center and throughout the Hospital System. Develop and update program policy in response to changing treatment regimens.
Coordinate and manage a system of peer support services for individuals who are infected with Hep C. Provide on-going services to clients enrolled in the Hepatitis C program to encourage retention in care. Supervise the Hepatitis C Program’s Linkage to Care Specialist and Peer Educator.


Conduct HCV rapid testing to targeted populations.
Coordinate the activities of GI consultant meetings with providers.
Exhibits critical thinking in the systematic assessment and formulation of individualized plans of care to meet patient’s needs.
Ensure all grant deliveries are met and that the program is meeting the funder’s Scope of Service and Workplan elements.
Maintains effective communication with staff, physicians, patients, and families, also within the unit and with other departments.
Demonstrates knowledge, respect, and sensitivity of patient’s rights. Serves as a patient advocate per policy.
Demonstrates valid provisional judgment and logical problem solving in the resolution of patient care or unit problems, appropriately informs Head Nurse and Director.
Performs unrelated duties in times of emergencies and assumes other responsibilities when assigned.
Responsible to develop and manage the Hep C program’s annual QI project and to actively participate in the Department’s Quality Improvement and Quality Assurance efforts as directed by their supervisor, including but not limited to participation on the Department’s Quality Improvement Committee.

Qualifications

Bachelor’s Degree in Human Services, Social Work, Psychology, Public Health or Counseling (required). At least three (3) years of work experiences in the field of HIV/Hep C, substance use, healthcare. At least three (3) years of supervisory experience in Prevention, Outreach, or Health Education or related field. Three (3) years’ experience in working with individuals with mental health and/or substance use. Must have a valid driver’s license and access to personal car for work related travel. Bilingual (English/Spanish) preferred.
