Clinical Coordination

  • Ensures communication and co-ordination of all the members of the multi-disciplinary team for transplant and ECMO service.
  • Ensure all necessary resources, equipment, and guidelines for the development of a transplant/ECMO program
  • Provides follow up and care across the continuum of care (pre, intra and post) for transplant and ECMO patients.
  • Active participation in daily multidisciplinary rounds offering clinical expertise in ECMO/Transplant management.
  • Provides long term follow up and management of transplant and ECMO recipient / patient
  • Responsible for accurate data collection and record keeping for       transplant program.
  • Responsible for data collection, accurate record keeping and data entry in the ELSO registry
  • Coordination and presentation of
          -potential transplant recipient cases at panel meetings
          -regular ECMO mortality and morbidity meetings
          -case presentations for MDT to learn and improve quality
  • Promote and practice evidence based medicine when caring for the potential organ donor.
  • Promote and practice evidence based medicine when coordinating the ECMO.
  • Provide a 24-hour consulting service to Nurses and other Healthcare professionals in relation to any organ donation queries.
  • Scientific early on-site evaluation of potential organ donors and ensure viability of referred organs for donor compatibility.
  • Obtain consent for organ retrieval from relatives of the potential organ donors and provide support and education to families of deceased potential donors regarding the donation process.
  • Maintain clinical support of a potential donor and family by partnering with the Clinical Psychologist to reduce donor anxiety and assist them through the procedure.
  • Communicate effectively and provide necessary information to the transplant medical team.
  • Liaise with all allied healthcare partners, the blood bank, laboratories and all other relevant departments to track samples, optimise progress and expedite availability of results for screening of potential donors.
  • Create a list of matching recipients once a donor is identified.
  • Collaborate with and refer suitable organs to other centres within the regions as well as nationally.
  • Coordinate the critical step by step prerequisites and in hospital arrangements required by the transplant team to manage the organ donor. Logistical requirements will include but not be limited to:
    • Theatre setup
    • Wards/unit communication
    • Anaesthetist consultation
    • Flight bookings (including ODF flights)
    • Transfer coordination according to flight protocol
    • Hospital and Nursing Manager communication
    • State Pathologist communication
    • Ambulance transfers
    • Support staff communication
  • Troubleshoot, locate and source equipment and resources unavailable at procurement centres.
  • Provide support and advice to ensure that the retrieval is efficient and professional which may include being in attendance during the theatre process.
  • Coordinate the clinical activities that are necessary to ensure the recovery and preservation of donated organs to ensure they remain viable for donation.
  • Assist with the packaging of procured organs and dispatch of organs to transfer centres.
  • Ensuring a holistic approach to care of patients within the ECMO/transplant program actively involving patient and family in decision making, management and follow up.
  • Collaborates with other ECMO/transplant centres and attends meetings to keep appraised on ECMO/transplant management and new technology.
  •   Promotion of ethical standards in all aspects of the program.

Education

  • Registered Nurse (RN) licensed to practice in SA SANC.
  • Intensive care qualification and a minimum of 2 years' experience in a Cardiothoracic facility.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.

Work experience

  • Minimum of 2 years of clinical nursing experience; prior experience in cardiovascular care or critical care essential.
  • At least 1 year of experience managing LVAD or heart transplant patients is highly desirable.

Knowledge

  • Strong leadership qualities including time management, decision-making, and organizational skills.
  • Excellent verbal and written communication skills with an emphasis on compassionate patient interaction.
  • Ability to work independently while fostering teamwork within a multidisciplinary setting.
  • Proficiency in using electronic medical records (EMR) systems.
