About the Role

Prairieland Service Coordination, Inc. is seeking a highly skilled and dedicated Case Manager to join our team in Decatur, IL. As a Case Manager, you will play a vital role in assessing individual needs and ensuring the provision of necessary services through effective communication, service coordination, and individual advocacy.

Key Responsibilities
  1. Assess individual needs and develop personalized service plans to meet their unique requirements.
  2. Coordinate with provider agencies to ensure seamless service delivery and monitor progress.
  3. Advocate on behalf of individuals and their families to facilitate access to eligible services and promote self-advocacy.
  4. Conduct regular visits, discovery, and personal plan processes to ensure ongoing support and monitoring.
  5. Maintain accurate and up-to-date individual files, adhering to program and agency guidelines.
  6. Provide problem-solving consultation with provider agencies, adhering to DHS Conflict Resolution guidelines.
  7. Enter billings daily into Birdseye to ensure timely and accurate payroll submissions.
  8. Participate in scheduled all-staff meetings and mandated training sessions to enhance professional development.
  9. Alert the Program Manager to problematic issues, trends, or service gaps, ensuring prompt attention and resolution.
Requirements
  1. A bachelor's degree in human services is required, with QIDP eligibility as established by the Department of Human Services.
  2. Excellent organizational, planning, and time management skills, with the ability to prioritize tasks and meet deadlines.
  3. A working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.
  4. Effective communication and interpersonal skills, with the ability to work collaboratively with professionals, paraprofessionals, and clients.
  5. Self-motivation and discipline, with the ability to work independently and with minimal supervision.
  6. Reliable transportation and a valid Illinois Driver's License, with a good motor vehicle record.
Physical Expectations
  1. Ability to read, write, and communicate effectively.
  2. Ability to ambulate, sit, and bend.
  3. Ability to carry files and lift up to 5 lbs.
