Coach : Integrated Care Management

**Minimum Requirements**:
- Have a grade 12 qualification
- A Tertiary qualification will be an added advantage
- Be able to develop a training needs analysis in line with our Performance Management System and ensure training interventions accordingly
- Be able to transfer skills and knowledge
- Develop and maintain Work Procedures and Standard Operating Procedures
- Effective and accurate recording of Performance discussions with all Coaches
- Train and ensure adherence to the Quality Management System
- Monitor service levels of all assigned processes using report/MS office in order to increase the service levels.
- Compile/collate/maintain Standard Operating Procedures and Work Instruction/processes using own knowledge and experience/job descriptions/group functions/legislation/MS Word in order to formalise group functions/ensure standardised work method.
- Be available to coachees on the floor as per group arrangement in order to ensure that the coachees knowledge and skills stay current.
- Liaise with coaches from other groups on a regular basis in order to resolve queries/maintains LA’s and a positive working relationship between various groups.
**Duties and Responsibilities**:
- Establish a coaching relationship with each assigned coachee in order to facilitate a productive working relationship.
- Identify and analyse areas for coaching in order to assist the development of coaches.
- Establish coaching goals and corrective/development action emanating from DPS’s/performance appraisals/quality assurance in order to ensure that coachees reach the required standard.
- Plan and prepare for GROW sessions in order to give coachees feedback on performance as per the set goals and or corrective/development action.
- Documents GROW, “Coaching on the Go” and One Minute Coaching sessions in order to keep accurate record of coaching which has taken place.
- Provide on the job training to coachees as required in order to ensure there are no gaps
- Provide training and mentoring for new clinical staff
- Provide needed training programs and maintain staff level of clinical Programs will include ICD-10, CPT-4 and PMB training.
- Evaluate/assess the quality of the consultants work using the Administration system/documented processes/published report in order to identify/address problems/shortcomings within the group.
- Communicate/give feedback to coachees during GROW sessions on quality assurance matters using the Administration system/prescribed quality evaluation forms/published reports/voice recorder in order to maintain a high quality of work in the group.
