Our client, in the Medical Field, has a Permanent vacancy for a Diabetes Care and Education Specialist.
The diabetes care and education specialist will render diabetes care support, coaching and education services to registered medical scheme beneficiaries by integrating the clinical care and self-management aspects for diabetes and cardiometabolic diseases. He/She will be an integral part of the clinical and disease management teams and will coach beneficiaries on how to set SMART goals.

Registered Nurse (additional post graduate certificate or diploma in counselling and/or psychology will be advantageous).
Successfully completed Advanced Diabetes Care course (CDE or DESSA).
A minimum of 3 years experience in Diabetes patient management and education whether within a Managed Care or private practice environment.

**Skills Required**:
Comprehensive knowledge, skills and experience in diabetes management.
Patient centric.
Excellent telephonic and written communication capability.
Excellent listening skills and an ability to relate to people.
Sensitivity and empathy.
Ability to cope with emotional situations.
Good organisational and task prioritisation skills.
High degree ofaccuracy and attention to detail.
Excellent report writing skillscase studies of high risk and/high-cost cases.
Must be able to work with a diverse patient population and have exceptional customer service skills.
Proficient with MS Office (Outlook, Word,Excel) (knowledge and understanding of medical scheme administration and/or managed care information systems will be advantageous).
Must be able to function within a team.

Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.
Advocates for and communicates about improved quality of care and outcomes for those living with bdiabetes and cardiometabolic conditions.
Performscomprehensive assessments for each patient including emotional and behavioralhealth.
Interprets personal health data, develops an individualised care plan based on the patients assessed needs and goals and promotes successful self-management.

Documents all individual contacts and outcomes datain accordance with the guidelines in a timely manner.
Identifies partially controlled patients and intervenes appropriately by liaising with the treating provider to manage the patients at risk.
Advocates for and supports technology-enabled diabetes education and care in individual and population health diabetes services.
Participates in meetings and gives assistance toclinical and disease management team members with hospital admissions and outreach efforts as appropriate.
Prepares reports/case studies as required.
Performs other duties as assigned within the Managed Health care environment.
