Fundamental to fulfilling the responsibilities of this post is the ability to respond flexibly, positively, and successfully to the ever-changing pressures and agendas faced by working with vulnerable adults & older people.
- The Social Worker is primarily responsible for providing case management services to patients/families.
- Primary duties include psychosocial assessment, care planning, implementation, discharge, resource identification and assistance with supportive counselling for grieving patients and family members.
- Outcome based treatment according to clinical social work services

Valid and current professional registration with the with SA Council for Social Service Professions (SACSSP).

Valid driver’s license and own vehicle **essential**
