**Job Advert Summary**:
**Purpose of the position**:
To provide professional and holistic social work services by assessing the needs of patients, families, carers and children with TB and/or HIV. The role involves implementing strategies and processes to meet these needs.

**Minimum Requirements**:

- Grade 12 or equivalent
- Certificate in Social Auxiliary Work from a recognized college or institution
- Minimum 2 years relevant experience
- Registration with the South African Council for Social Service Professions (SACSSP)
- Proficiency in English and isiXhosa languages
- Clear criminal record
- Computer literate in MS office advantageous

**Duties and Responsibilities**:

- Assists in the coordination of comprehensive psychological assessments of patients and their families at Anti Retroviral Clinics/ Mobile
- Educates patients and their families
- Informs patients and their families on eligibility criteria for social support grants
- Networks with all relevant stakeholders and implements an effective referral system to ensure effective service delivery
- Lobbies and advocates for patient’s needs
- Provide appropriate interventions and/or feedback at the case management meeting on the needs arising
- Follows up on referrals from hospital staff and/or teams
- Submits monthly reports to the supervisor
- Records and maintains accurate and detailed patient information and interventions and keeps statistics in order to submit as required
