Responsibilities

The Patient Navigator is tasked with delivering client-centered support, education, and connections to care services for patients within the hospital's Emergency Departments. Collaborating with existing HIV/Hepatitis C treatment and prevention services, the Patient Navigator will coordinate and advocate for services that foster positive health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES

- Conduct in-person outreach, education, and linkage to care services for individuals visiting the hospital's Emergency Departments.
- Assist staff and patients in engaging with HIV/HCV/HBV testing during their visit to the Emergency Departments.
- Provide supportive counseling and connections to services related to new HIV/HCV diagnoses, disease progression, available medical treatments, adherence, and other requested information.
- Offer patient-centered referrals and follow-ups to appropriate resources based on the patient's health and behavioral needs concerning HIV, Hepatitis B, and C.
- Utilize available prevention, primary/specialty healthcare, and behavioral health services to increase the number of individuals diagnosed with HIV and/or Hepatitis C who are linked to necessary services.
- Build relationships with internal and external service providers to ensure timely access to services for patients.
- Monitor and track all referrals, remind patients of upcoming appointments, and assist with rescheduling as needed.

- Develop a Brief Action Plan with patients to support self-management skills.
- Assess patient risk factors and implement a harm reduction approach in advocating for services.
- Document all interventions conducted with or on behalf of the patient in the hospital's EMR (Meditech) accurately and promptly.
- Ensure the confidentiality of patient healthcare and personal information is maintained at all times.
- Safeguard electronic devices and passwords, ensuring appropriate consent forms are signed and updated as required.

Qualifications

QUALIFICATIONS
1. Minimum of 12 months' experience in patient navigation or a related field.
2. Relevant education/experience/personal knowledge concerning HIV, Hepatitis B/C, substance use, and harm reduction.
3. Specific language skills may be required, such as fluency in Spanish.
4. Proficient in computer operations.
5. Strong interpersonal skills with effective verbal and written communication abilities.
6. Demonstrates confidence and sound judgment in actions.
7. Capable of planning, organizing, developing, and implementing goals, objectives, policies, and procedures necessary for quality care.
8. Ability to recognize problems and collaboratively work towards solutions.
9. Valid driver's license and access to a personal vehicle for work-related travel.

EDUCATION
High School diploma or GED
Associate degree in human services or social work
