About the Role:
The Practitioner - Residential Care is a critical position within our organization, responsible for providing case management to children or young people within residential care services or programs. This role requires a high level of skill and expertise in developing individual strength-based care plans, assessing risks and needs, and creating positive behavior support plans.
Responsibilities:
- Develop individualized care plans that prioritize the strengths and needs of each child or young person.
- Collaborate with the care team to assess risks and needs, and develop strategies to address them.
- Create positive behavior support plans, crisis management plans, and daily/weekly routines that promote the well-being and safety of children and young people.
- Support children and young people transitioning into and out of services, and work with support networks to ensure successful transitions.
- Effectively communicate with internal and external stakeholders, including families, carers, and other professionals.
- Develop cultural pathways for Aboriginal and/or Torres Strait Islander children and youth, and work to promote cultural awareness and understanding within the organization.
- Participate in community engagement and maintain administrative requirements, including accurate record-keeping and reporting.
Qualifications and Requirements:
- Degree in Health, Human Services, Behavioural Science, Social Work, Psychology, or a related field.
- Current Blue Card and Licenced Care Service (LCS) certification.
- Current Queensland Driver Licence.
- Ability and willingness to travel within the service region and to work outside of standard business hours, as required.
About You:
- Applied knowledge and understanding of youth justice and child protection frameworks, including the Youth Justice Act and Child Protection Act (1999).
- Understanding of child and adolescent development, including the impacts of trauma, attachment disorder, and grief and loss.
- Ability to assess complex to extreme needs of young people and develop strength-based care plans that target areas for development.
- Highly developed problem-solving, decision-making, and negotiation skills, with the ability to respond appropriately in crisis situations.
- Able to work effectively in a team and independently, with strong verbal and written communication skills.
