About the Position

The role of a Support Coordinator is to coordinate the supports for NDIS clients and assist them in achieving their goals outlined in their NDIS funded plan. This involves proactively identifying, developing, coordinating, and facilitating access to supports and services alongside allocated NDIS clients.

Key Responsibilities
  • Work with clients, families, and carers to identify and access opportunities for skills development, education, and employment.
  • Support and encourage clients to achieve their goals and provide support coordination services in line with their NDIS plans.
  • Develop networks and collaborate with stakeholders to proactively identify, plan, implement, and evaluate supports.
  • Prepare and maintain up-to-date and accurate case management notes and reports using clear and concise language on client progress.
  • Liaise with a wide range of stakeholders, community services, government agencies, customers, and service providers.
  • Attend and contribute to team meetings, discussions, and planning. Generate ideas and options while maintaining flexibility and receptiveness to change and alternatives.
Required Skills and Qualifications
  • A highly organised person with excellent oral, written, and interpersonal communication skills.
  • Able to work well both independently and cooperatively as a member of a team.
  • A person who displays self-discipline with the ability to avoid distractions and diversions in order to manage their own time effectively.
  • Analytical and creative skills are necessary to analyse individual needs and identify, measure, and report outcomes.
  • A positive and contemporary values towards people with disabilities.
  • Able to interact, communicate, and engage effectively with diverse cultures.
