The Right to Care organization is seeking a skilled Patient Care Navigator to join our team. As a Patient Care Navigator, you will play a vital role in ensuring that patients newly diagnosed with HIV and/or TB or STI are linked to care and commenced on ART as applicable.
About the Role:
We are looking for an experienced professional who can work effectively with communities, home-based care, and community-based organizations to improve linkage to care.
Key Responsibilities:
- Ensure timely referral of patients from informal settlement programs to DOH facilities for further management.
- Collaborate with facility staff to implement services that increase retention in care of clients initiated on ART.
- Develop and maintain relationships with community leaders and healthcare providers to promote effective care coordination.
Requirements:
- Matric (Senior Certificate)
- 2 years' experience in TB or HIV field
- Computer Certificate
