We are looking for an experienced Specialised Healthcare Coach x2 to join our team at Momentum Health. In this role, you will be responsible for managing clinical risk of patients through the authorisation of clinically appropriate treatment and outbound interventions. You will work closely with patients, providers and Disease Management companies to ensure that patients receive the best possible care while minimising costs.

Key Requirements:
  • A degree or diploma in Nursing (General, Psychiatric, Community) and Midwifery is required.
  • Current registration with SANC, on application is necessary.
  • Minimum of 3 years clinical experience essential, with a minimum of 3 years' experience in Managed Health Care Industry.
  • Experience in the management of chronic diseases is essential, with experience in renal dialysis, organ transplant, psychiatry or oncology being highly advantageous.
