The Nurture Health Group requires an individual to fill the dual role of Clinical Assessor who will be based at our Nurture Beacon Bay physical rehabilitation facility in East London reporting to the Rehab Centre Manager.
Minimum Salary: R21 878.86 Cost to Company
**REQUIREMENTS, EXPERIENCE AND QUALIFICATIONS/TRAINING**
- Allied Health Professional (HPCSA registered) or Registered Nurse (SANC registered)
- 2 years of clinical experience.
- Experience in other fields of rehabilitation such as management, task teams, or policy/process implementation will be an advantage.
- Experience working with ICD 10 coding advantageous. Excellent and creative marketing skills.
- **KEY COMPETENCIES/OUTCOMES**
- To oversee the patient journey from acute to post-discharge through the completion of pre-admission assessments; in-hospital treatment and post-discharge follow-up.
- To deliver quality preventative, curative, and psychosocial therapy interventions.
- Intense and directed liaison with families, patients, and referring doctors to ensure values-based care is being provided.
- Advocate on behalf of the patients to ensure policies and services serve their best interests through assisting with public relations, communication, and ethical marketing.
- Coordinate Admission and discharge management and facilitate continuum of care (from inpatient to outpatient care).
- Ensure patients’ objectives and weekly tasks are met to achieve a successful discharge across all functional areas.
- Must be willing to learn and adapt to the Nurture Rehabilitation system, training will be provided.
- Must enjoy working with a full interdisciplinary team of therapists, nursing staff, admin support and doctors.
