**PRIMARY ROLE**

**Patient Needs**
- Upon receiving patient information for admission, make contact with the patient/family to establish patient needs.
- Advise patient/family of what is expected at Spescare and what they need to bring along.
- Wellness rounds.

**Collaborate with MDT members**
- Collaborate with professionals to make a holistic assessment of patient’s social circumstances and socio-emotional needs, medical and physical conditions to establish patient’s needs.
- Report writing on patient’s home circumstances, support-systems, socio - emotional functioning, discharge plan etc.
- Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficiency.
- Continuously evaluate and modify treatment plans, in conjunction with multi discipline team to comply with changes in clients' status.
- Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
- Identify environmental impediments to client or patient progress through interviews and review of patient records.
- Ensure adequate paperwork and record keeping of own interventions.
- Advise and Direct other workers providing ultimate care to patients.
- Provide training where needed.
- Arrange Family Meetings where needed and ensure adequate notes are kept.
- Plan and coordinate a safe discharge.
- Reporting and Administration.

**SECONDARY ROLE**
- Counselling, education to patients and their families to help them understand their medical condition and the effects thereof, helping them to adjust and cope with changes, challenges, and emotions.
- Counsel clients and patients in individual and group sessions to assist patients with their recovery plan, recover from illness, resolve crises, and adjust to life.
- Support - Staff Wellness, Staff Satisfaction, Plan and coordinate special days, Participate in MANCO Meetings.

**QUALIFICATIONS**
- Bachelor's degree is the minimum formal education required for this position.

**SKILLS**
- Oral Comprehension — the ability to listen to and understand information and ideas presented through spoken words and sentences.
- Oral Expression — the ability to communicate information and ideas in speaking so others will understand.
- Written Comprehension — the ability to read and understand information and ideas presented in writing.
- Problem Sensitivity — the ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing there is a problem.

**CERTIFICATES, LICENSES, REGISTRATIONS**
- Registration with South African Council for Social Service Professions (SACSSP)
- Registration should be renewed yearly and it the responsibility of the Care Coordinator.
- The Care Coordinator is responsible for CPD upkeeping and training.
- Proof of registration must be submitted to manager.
