Position Title: Chronic Disease Care Coordinator Program: PHC Location: Alice Springs Classification: WDNWPT EA: Clinical Non-dialysis Level 1, pay point 3 Responsible To: PHC Manager Position Summary The Chronic Disease Care Coordinator is responsible for offering extra support to high-risk patients with Chronic Diseases. The objective is to enhance their access to healthcare services and promote positive health outcomes. This role includes helping patients set goals, identify the necessary health and support services to achieve those goals, navigate the healthcare system, and advocate for their individual needs. About Purple House Our mission is to improve the lives of people with renal failure, reunite families and reduce the impact of kidney disease in our communities. We are run by Aboriginal people for Aboriginal people and work to provide culturally appropriate dialysis services in remote communities, helping people to get home to country and family. Demonstrate cultural awareness and knowledge of cultural safety and Indigenous community control. Maintain confidentiality of information about clients, other staff members or community members. Work to achieve standards of best practice and promote continuous quality improvement. Demonstrate safe work practices and promote a safe work environment. Functions Collaborate with all employees of WDNWPT and other health providers to ensure continuity of holistic, quality care for patients in line with the organization’s strategic goals. Manage the healthcare needs for a caseload of patients with multiple, complex, chronic diseases, specifically those transitioning between urban and remote areas, as well as those who have newly commenced on dialysis. Facilitate access for our patients to internal GP, allied health, and specialist clinics and services. Schedule and support patients in attending external healthcare appointments as required. Build strong relationships and maintain close communication with external patient care services to facilitate a multidisciplinary approach to healthcare for all patients. Be flexible and generous in providing healthcare and ensuring accessibility to the service. Utilize a recall system to ensure patients’ healthcare needs are met in a timely manner. Acknowledge the social determinants of health, and working closely with Purple House Malpa team to provide a wholistic approach to care goals Advocate for patients and protect their individual rights. Maintain patient records to a high standard, including progress notes, records of communication regarding patients, and updates to medical history and demographic information. Liaise closely with your direct manager for assistance and advice when needed, and participate in regular team meetings. Promote a safe work environment in accordance with legislative requirements, adhere to work health and safety guidelines, and report all incidents and near misses. Perform other duties as required within your trained capacity and level. About You You have a patient centred focus and eye on our goal of getting people home on country.
