Position Title: Primary Health Care Aboriginal Patient Liaison Program: Primary Health Care Location: Alice Springs Classification: WDNWPT Aboriginal Corporation EA: Community Services Worker – Level 2 Responsible To: Primary Health Care Manager Position Summary The Aboriginal Patient Liaison (APL) within the Purple House Primary Health Care team works collaboratively with the internal Primary Health Clinical, Allied Health, and Intensive Support groups, along with external health services, to assist patients in accessing health care, education, and social support, while providing advocacy and maintaining a culturally safe experience. Special conditions 15 hours/week casual or part-time contract New appointments are probationary for the first six months. About Purple House Our mission is to improve the lives of people with renal failure, reunite families and reduce the impact of kidney disease in our communities. We are run by Aboriginal people for Aboriginal people and work to provide culturally appropriate dialysis services in remote communities, helping people to get home to country and family. Demonstrate cultural awareness and knowledge of cultural safety and Indigenous community control. Maintain confidentiality of information about clients, other staff members or community members. Work to achieve standards of best practice and promote continuous quality improvement. Demonstrate safe work practices and promote a safe work environment. Functions Facilitate the delivery of internal Primary Health Care services for Purple House serviced patients. Work closely with the Purple House Malpa teams to contribute to a holistic approach to Primary Health Care Work collaboratively with external health and other services, as required. Ensure regular communication with all other members of the Primary Health Care Team regarding daily tasks, and any patient issues. Provide Health promotion and preventative health care advice to all patients, within scope of practice, as directed by health care staff. Provide well-being checks on patients of concern, as directed by clinical staff. Advocate for patients and protect the rights of the individual Assist in the transport of patients to both internal and external health and other service appointments. Provide continuous liaison with Purple House serviced patients to ensure they are well informed about health care requirements and upcoming appointments. Ensure documentation of all communication with or regarding patients and maintain patient records to a high standard. Liaise closely with direct manager for assistance and advice when required and partake in regular team meetings. Promote a safe work environment in accordance with legislative requirements, adheres to Work Health Safety guidelines, and reports all incidents and near misses. Qualifications, Skills & Experience Proven ability and desire to work with the multidisciplinary team to drive positive change in health outcomes. Understanding and experience working with vulnerable patients Well developed written, verbal and interpersonal skills Ability to communicate effectively and safely in a cross-cultural environment Current NDIS/Working with Children clearance Current Criminal History Check Basic Life Support certificate NT Driver’s licence #J-18808-Ljbffr
