This will include screening and assessment, and referral to social worker for therapeutic social work services, within the C&T Programme.


Minimum Requirements:

  • Grade 12 or equivalent
  • Certificate in Social Auxiliary Work from a recognised college or institution
  • Minimum of 2 years' related experience

SACSSP (South African Council for Social Service Professions)

  • Proficiency in English and isiXhosa languages
  • Valid code 8 driver's licence will be advantageous

Duties and Responsibilities:

  • Conducts comprehensive psychosocial assessments and household profiling
  • Provide minimum package of communitybased care and refer to social worker /facility for continuum of care
  • Follow up on service beneficiaries such as children, adolescents and youth to provide relevant psychosocial support
  • Provides mental health and substance abuse screening and refer to social worker and other external networks
  • Educates patients and their families
  • Informs clients and their families on eligibility criteria for social support grants
  • Works under supervision of social worker in a multidisciplinary team
  • Networks with all relevant stakeholders and implements an effective referral system to ensure effective service delivery
  • Lobbies and advocates for patient's needs
  • Assist in implementation of support groups, adherence and youth care clubs
  • Attends training and workshops as part of staff development
  • Submits monthly reports to the supervisor
  • Records and maintains accurate and detailed patient information and interventions
  • Keeps patient information confidential and facilitate disclosure
