Patient Care Manager

The Patient Care Manager will oversee the day-to-day operations of our 12 Bed Intermediate Care Facility, Home Care Department and the Psycho-Social Department.
They are responsible for managing and leading a dedicated interdisciplinary team of sisters and social workers, ensuring they work together effectively to provide the best possible care for patients with chronic and palliative illnesses. Additionally, they participate in the strategic growth and development of the organisation.
**Requirements**:
- Professional registration with Nursing Council
- 3 years minimum experience in Palliative Care (or willing to do training)
- Initiative, drive and enthusiasm are essential qualities
- Minimum 2 years experience in project or team management
- Excellent planning, organizational and administrative skills
- Excellent people skills, good communication skills
- Fluent in English and Afrikaans, knowledge of Xhosa an advantage
- Valid driver’s license
**Responsibilities**:
- Manage & lead our dedicated interdisciplinary team of sisters and social workers
- Supervise and support all professional and para-professional nursing staff
- Ensure continuity of care for all our patients - chronic and palliative
- Maintain our COHSASA accredited quality of nursing care services
- Participate in the strategic growth and development of our Hospice
- Working hours are from 08:00 - 16:30
