**Key Responsibilities**:
1. **Individual Assessment and Care Planning**:

- Conduct comprehensive assessments of residents' social, emotional, and psychological needs.
- Develop individualized care plans in collaboration with residents, their families, and the interdisciplinary team.
- Regularly review and update care plans to ensure they align with residents' evolving needs.

2. **Emotional Support and Counseling**:

- Provide emotional support to residents and their families as they navigate the challenges of dementia.
- Conduct individual and group counselling sessions to address psychological and emotional concerns.
- Facilitate support groups for residents and families to encourage mutual support and understanding.

3. **Advocacy and Collaboration**:

- Serve as an advocate for residents' rights and well-being within the facility and the broader community.
- Collaborate with healthcare professionals, including nurses, doctors, and therapists, to integrate social work principles into the overall care plan.
- Work closely with other staff members to create a cohesive and supportive living environment.

4. **Education and Training**:

- Provide educational resources and training to residents, families, and staff on dementia-related topics.
- Foster a culture of understanding and empathy by promoting awareness and knowledge about dementia within the facility.

5. **Crisis Intervention**:

- Respond to crisis situations promptly and effectively, providing emotional support and practical assistance.
- Collaborate with the team to implement crisis intervention strategies that ensure the safety and well-being of residents.

6. **Documentation and Reporting**:

- Maintain accurate and up-to-date records of assessments, care plans, and interventions.
- Prepare reports for management and participate in case conferences to discuss residents' progress and challenges.

7. **Communication with Residents Family**

**Qualifications and Experience**:

- Bachelor’s degree in social work
- Registered with the South African Council of Social Service Professions (SACSSP)
- Experience - 10 years+
- Preferred experience working with individuals with dementia and their families.
- Knowledge of dementia care best practices and current research.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Compassion, patience, and a genuine commitment to improving the lives of individuals with dementia.

**Salary**: R30,000.00 - R35,000.00 per month
