This is a unique opportunity for an experienced professional who is passionate about holistic, patient-centered care and values the flexibility to shape their own schedule.

At **Empowered to Thrive**, we support individuals navigating complex medical recoveries—including **Neurological, Orthopedic, Surgical, and Respite care**—by offering critical psychosocial services that promote well-being and safe discharge planning. Our social workers play a vital role in supporting patients and families through **counseling, mediation, case coordination, and stress-reducing interventions** to ensure optimal engagement in the rehabilitation journey.

**Responsibilities**:

- Assess Care needs
- Assist families to implement recommended care to ensure safe discharges are in place.
- Connect patients to relevant resources such as care agencies, frail cares, day clinics, palliative care resources etc.
- Make referrals to appropriate service providers such as psychologists, Department of Social Development etc.
- Provide short term counseling
- Provide mediation to patients and families
- Provide practical assistance to patients to reduce stressors in order to participate in their rehabilitation.

**What We’re Looking For**:

- ** SACSSP Registration** (current and valid)
- ** Professional Indemnity Insurance**:

- Training and experience in **Palliative Care** (essential)
- Additional experience in **Oncology**, **Curatorship**, or **Geriatric/Frail Care** (advantageous)
- Proven ability in **Hospital Social Work** and **Case Management**:

- Strong **report writing** and **time management skills**:

- Comfortable working **flexible hours within normal business times**:

- Willingness to **travel for outpatient consultations**:

- Ability to collaborate effectively with an **interdisciplinary care team
