They are seeking a highly experienced and mature Care Manager to lead our team with empathy and expertise.

**Key Responsibilities**:
**Team Leadership**: Manage and mentor a team of caregivers and administrators, ensuring high standards of care and service delivery.

**Patient Assessments**: Conduct thorough patient assessments to create personalized care plans that meet individual needs.

**Care Coordination**: Oversee the scheduling and coordination of care services, ensuring that all patient requirements are met efficiently.

**Onboarding and Support**: Onboard new patients, ensuring they understand the care process and feel supported. Provide ongoing support to both patients and their families.

**Quality Assurance**: Monitor and evaluate the quality of care provided, implementing improvements where necessary to maintain high standards.

**Compliance**:
Ensure compliance with all relevant health and safety regulations, industry standards, and company policies.

**Budget Management**: Manage the care budget, including resource allocation and cost control, while ensuring optimal patient care.

**Liaison with Medical Aids**: Work closely with medical aids to ensure patient care plans are covered and manage any related administrative tasks.

**Crisis Management**: Respond promptly to emergencies or urgent patient care needs, providing after-hours support as required.

**Reporting**:
Prepare regular reports on patient care, team performance, and operational efficiency for senior management.

**Requirements**:
**Experience**: Minimum 5-10 years of experience in a similar managerial role within the healthcare or home-based care industry.

**Education**:
Degree or Diploma in Nursing, Healthcare Management, or a related field.

**Skills**:

- Strong leadership and team management abilities.
- Excellent interpersonal and communication skills.
- Deep understanding of patient care and the home-based care environment.
- High proficiency in budgeting, reporting, and administrative management.
- Ability to work under pressure and manage crises effectively.
- Strong knowledge of compliance and healthcare regulations.
- Proficiency in computer software including Pastel and Excel.

**Other**:

- A valid South African driver’s license and reliable vehicle are essential.
- Flexibility to manage after-hours care and emergencies.

**Salary Range**:
R25,000 - R40,000 per month, depending on experience and qualifications.

**How to Apply**:
