We are seeking a skilled and experienced Clinical Care Coordinator to join our team at Right to Care. As a key member of our community-based service team, you will play a vital role in delivering high-quality clinical care services to individuals and communities.

Job Summary

The successful candidate will be responsible for planning, coordinating, and implementing community-based clinical care programs, including clinical care, health promotion, and adherence support. You will work closely with our community health workers, patients, and other stakeholders to deliver effective clinical care services that meet the needs of our clients.

Main Responsibilities

  • Clinical Care: Administration of immunization in the community, administration of Vit A and deworming, provision of Sexual Reproductive Health services, and conducting on-the-spot HIV testing when needed.
  • Basic Care and Rehabilitation: Assessment of referred clients, determination of type of nursing care, provision of nursing care plans, and allocation of CHWs to clients in the catchment area.
  • Health Promotion and Wellness: Supervision of wellness campaigns, identification of clients in need of Child Health services, and planning meetings with Operational Manager, Wellness PN, and other NGO projects.
  • Adherence Support: Liaison with PHC services, provision of assistance at Alternative Distribution Sites, and education of patients on medication intake and lifestyle changes.
  • HUMAN RESOURCE DEVELOPMENT: Identification of training needs, completion of an annual in-service training roster, and coordination of formal CHW training.
  • Quality Assurance: Conduct of monthly Quality Assurance visits, analysis of monthly data, and implementation of quality improvement plans.
  • Recordkeeping and Data Collection: Assessment of availability and completeness of standardized forms, explanation of data collection tools to CHWs, and submission of verified weekly and monthly data.
  • Administration: Attendance of Departmental meetings, implementation of 2-weekly or monthly meetings with PHC facilities, and submission of annual leave end of June.
