**Job Overview:**
We are seeking a highly skilled and compassionate Renal Care Coordinator to join our team at Life Healthcare Group. The successful candidate will be responsible for facilitating the enhancement of holistic disease management for patients with kidney disease.
**Key Responsibilities:**
- Act as the primary point of contact for patients with kidney disease, providing education and support throughout their care journey.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and efficient use of resources.
- Develop and implement personalized care plans tailored to each patient's unique needs and goals.
- Monitor and track patient outcomes, identifying areas for improvement and implementing changes as needed.
**Requirements:**
- Grade 12 - Matric certification.
- Nursing qualification (SANC) or HPCSA registration appropriate to relevant medical qualification.
- Minimum 3-5 years' experience in renal nursing or related field.
- Knowledge of hospital patient services and billing processes.
- Computer proficiency and experience with hospital information systems.
**Competencies:**
- Strong communication and interpersonal skills.
- Able to work autonomously with minimal supervision.
- High level of sensitivity and diplomacy.
