Job Summary
We are looking for a dedicated and detail-oriented individual to join our team as a Facility Care Specialist. The successful candidate will be responsible for ensuring that patients diagnosed with HIV and/or TB are promptly linked to care and commenced on ART as applicable.
Main Responsibilities
- Develop and maintain strong relationships with community healthcare workers, providing them with training and support to effectively identify and refer patients in need of HIV and/or TB services.
- Collaborate with facility staff to enhance service delivery, improve patient retention, and increase retention in care of clients initiated on ART.
- Conduct regular monitoring and evaluation of linkage to care initiatives, identifying areas for improvement and implementing changes as needed.
Qualifications and Experience
- Degree in a relevant field, such as health sciences or social work.
- At least 2 years' experience in TB or HIV case management.
- Familiarity with healthcare systems and policies, with the ability to navigate complex bureaucratic processes.
