The Client Advocate will be responsible for working closely with clients, families, and caregivers to develop and implement personalized plans of care that promote health, wellness, and independence.
Main Responsibilities- Client Assessment: Conduct comprehensive assessments to identify client needs and preferences
- Service Coordination: Develop and implement individualized plans of care that meet client needs and goals
