Main Responsibilities

The successful candidate will be responsible for facilitating the expansion and coverage of our organization through mapping and identifying hotspots in the community. This will involve strengthening collaborations with partners, community, and stakeholders to improve treatment uptake, care, and support between the communities, community-based structures, and identified health facilities.

Key Performance Areas
  1. Conduct microplanning activities, including hotspot mapping and site profiling, to identify areas of need and develop targeted interventions.
  2. Identify and engage community advisory board (CAB) members to ensure active representation and engagement by the target population.
  3. Plan and implement events and campaigns, in collaboration with the outreach team, to promote health education and awareness.
  4. Liaise with other stakeholders on healthy lifestyle programs and collaboration to improve overall health outcomes.
  5. Contribute to the evaluation of the process of community mobilization to ensure effectiveness and efficiency.
  6. Work closely with the district and sub-district teams to document and respond to issues arising from the community.
  7. Liaise with sub-partners and other community-based organizations to ensure timely interventions for human rights and gender-based violations.
  8. Continuous engagement with the public health clinics, Department of Social Development, and Police in the area to ensure good working relationships and raise awareness of the need for services.
  9. Support the monitoring and evaluation system to measure the coverage and impact of the community intervention.
  10. Provide HIV Testing Services (HTS) as per DoH policy guidelines and protocol.
  11. Provide accurate health information to the target population, especially on HIV/AIDS, PrEP, STIs, TB, and contraception, and promote healthy behaviors/habits.
  12. Track, trace, and link individuals to care.
  13. Provide and promote ART adherence support and follow-up.
  14. Provide and promote psychological support, referral, and follow-up.
  15. Provide accurate human rights information to the target population.
  16. Human Rights Defense and Support, or referral and follow-up.
  17. Plan and facilitate the Risk Reduction Workshops and or Creative Space workshops and identify themes and topics from outreach activities.
  18. Distribute safer sex commodities (male condom; female condom; lubricant; IEC material).
  19. Conduct condom demonstrations and promote correct and consistent condom use.
  20. Supervise CHWs and Team Leaders during outreach activities.
  21. Contribute to the compilation of monthly and quarterly reports for management and donors on community engagement, including compiling correct stats.
  22. Respond to emails from internal and external stakeholders in a timely manner.
  23. Take ownership and accountability for tasks and demonstrate effective self-management.
  24. Maintain a positive attitude and respond openly to feedback.
  25. Participate in ongoing training and development activities to drive career development.
Requirements

The ideal candidate will have a Grade 12 qualification, a valid driver's license, and fluency in English and other African languages. A minimum of 2 years of experience in community health development and programs is required.

A tertiary diploma or degree in a relevant field, such as health, education, public relations, or marketing, is desirable. A certificate in HIV Management is also an advantage. Knowledge of Microsoft Word and Excel, electronic data collection, and experience with community engagement and marketing new services are highly valued.

The successful candidate will be a team player, able to manage their own time, work independently, and as part of a multi-disciplinary team. They will be adaptable, approachable, empathetic, and willing to take initiative. Good communication skills, both verbal and written, and the ability to network with various stakeholders are essential.

Able to work independently and as part of a team, the ideal candidate will be tactful, respectful, and maintain confidentiality. They will be able to provide accurate health information, promote healthy behaviors, and track, trace, and link individuals to care.
