We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Hospice LPN, providing patient-centered care in Oklahoma and Arkansas.

About the Job: As a Hospice LPN, you will be responsible for providing skilled nursing care to patients in their homes, ensuring their comfort, dignity, and well-being.
You will work closely with the interdisciplinary team, including physicians, nurses, social workers, and spiritual care coordinators, to develop and implement individualized plans of care that meet the unique needs of each patient.
Responsibilities: The successful candidate will be responsible for:
- Providing skilled nursing care to patients in their homes;
- Assessing patients' conditions and determining when consultation is required;
- Developing and implementing individualized plans of care;
- Working collaboratively with the interdisciplinary team;
- Documenting patient information and communicating with healthcare professionals.
Qualifications: To be considered for this role, you must have:
- Current Arkansas/Oklahoma Compact LPN license without restriction;
- Graduate of an accredited school of nursing;
- At least one year experience in home health, hospice, gerontology, geriatrics, acute care & care of the terminally ill;
- Reliable transportation and a current driver's license with liability insurance.
