About the Role

We are seeking a highly skilled Clinical Care Coordinator to join our team at The Career Network.

Job Summary

The successful candidate will be responsible for ensuring effective and efficient monitoring of referrals, admissions, discharges, compliance, and patient movements within a multidisciplinary team. This role requires strong communication skills, clinical knowledge, and attention to detail.

Key Responsibilities
  • Clinical Function: Referral process
  • Participate in new on-site admission screening meetings
  • Develop contacts with referring partners to optimize continuum of care
  • Promote good interdisciplinary relationships with referring sources
  • Ongoing verbal and written information sharing with referring facilities
  • Proactively report bed availability to referring facilities weekly
  • Screen referrals to ensure patients meet admission criteria
  • Ensure documentation is in place for referrals to be processed
  • Monitor prospective patient information distribution and MDT minutes
Admission and Discharge Process
  • Optimize continuum of care through effective patient management
  • Enhance patient flow management by improving admissions process
  • Collaborate with clinical team for comprehensive discharge planning
  • Ensure patient safety and efficient bed utilization
  • Manage patient admissions and emergency transfers
Administrative Function
  • Monthly reports on patient documentation and record-keeping
  • Weekly updates on bed capacity and availability
  • Development, monitoring, and implementation of SOPs
Servcies Quality and Compliance
  • Understand quality importance in daily work
  • Participate in quality measures to ensure high-quality service delivery
  • Uphold company vision, mission, and values
  • Evaluate and coordinate care provision to ensure it is safe, timely, effective, efficient, equitable, and patient-centered
Strategic Role and Function
  • Form part of the patient safety committee
  • Implement strategies for control and evaluate standards of service delivery
  • Take responsibility for effectiveness of service delivery
Required Qualifications
  • Degree or Diploma in Nursing/Allied Health
  • Current registration with SANC/HPCSA
  • Understanding of legislation and ethical requirements
  • Be clinically sound/knowledgeable to judge and facilitate appropriate admissions
