NACOSA is looking for suitably qualified **Health Connectors** within the Moretele sub-district (NW) to support the implementation of a youth health programme called **_Imagine: Social Outcomes Based Contract_**. Imagine is focused on improving sexual and reproductive health outcomes amongst Adolescent Girls and Young Women in Six selected Moretele High Schools over a 2.5-year period.

The purpose of the **Health Connector** positions will be to support the integration between DoH Clinic Facilities and the Imagine Clinical Teams in the school environment, ensuring that there is successful referral and linkage to clinic facilities, as well as effective data capture in the Imagine Information Management system as well as the DoH data systems such as Tier.net and Labtrack. The **Health Connectors** will play a critical role in maintaining good cooperative relations between Imagine Clinical Teams and the relevant DoH Clinic Facilities.
- Ensures that proper linkage to care of all clients is done according to DoH guideline.
- Ensure that all clients who tested HIV positive are linked into care and keep record of how many are initiated on ART.
- Accurately record and capture services rendered to the female learners’ using the digital Imagine Information Management system.
- Accurately record and capture services rendered using the DoH Clinic Facilities Imagine Information Management system such as TIER.net etc.
- Conduct tracing of patients who have missed their appointments or defaulted from care.
- Assist in maintaining good clinic flow through structured planning and coordination with all Imagine staff on a daily basis.
- Contribute to the development of a smooth service delivery process within the Imagine teams and seamless referrals into Clinic Facilities and or Hospitals.
- Maintain accurate and well organised client records throughout the programme.
- Maintain high standards and quality client experiences through ongoing discussion, consent, confidentiality and professionalism.
- Keep records of number of clients offered HTS, tested, tested HIV positive, linked to care, etc
- Attend DoH Clinic facility meetings as required to discuss HTS/Pregnancy related issues
- Attend medical education and advocacy talks on Comprehensive Sexuality Education (CSE) in advocacy workshops with learners and parents at each school, aspiring towards maximum information coverage and acceptance and understanding of the Imagine Programme.
- Provide scientifically accurate information on sexual and reproductive health products and services & explain the process of accessing those products and services in a simple way.
- Provide detailed education on sexual reproductive health services to clients. Including the step by step processes involved in prevention, treatment and management according to need.
- Provide Family Planning Advocacy and information on different contraceptive products.
- Become a consistent source of trusted medical information and help in the school community.
- Minimum Matric / Grade 12 / NQF4
- 2 - 3 year's relevant experience as a HAST Coordinator working in a HAST programme or health facility
- ATTIC Certification
- Successful completion of basic counselling course
- Completion of HIV/AIDS courses or certifications
- Practical experience working in the HIV/AIDS and Education field within the NGO and NPO sector working with AGYW
- Read, write, and speak in two official South African languages, of which one must be English.
- Ability to effectively present information and respond to questions
- Good record keeping and data capture skills with a very high degree of accuracy and consistency.
