**Work Description**
The Case Facilitator is responsible for providing basic patient level support to all HIV positive newly diagnosed patients, patients already on HIV treatment, those beginning their treatment journey, PrEP seeking clients and those who are on PrEP and need to visit for their follow ups
**Technical Work Responsibilities**
Responsible in peer education / recruitment
Maintaining adherence in quality control testing services
Supervising lay counsellor on duties
Identifying MSM / Trans gender high transition area
Organising M group and core group meeting
Ensuring target meeting targets
**Duties**:
Liaise with stakeholders and contribute towards the achievement of Company and Divisional goals
Effective communication with relevant stakeholders regarding constraints and appropriate resolutions
Maintain favourable relations with internal and external stakeholders
Navigate and access the health care system and conduct face to face patient follows ups to appointments, tracking patients who have missed appointments and provide education and information around treatment and care of HIV and TB
Complete the Case facilitation register and review it daily.
Call Patients within 48 hours following initiation for support.
Remind patients of their forth-coming appointment dates, working in partnership with clerks for booking of appointments.
Follow up if patient attended facility appointments (7 days, 30 days & any other)
Link patient with a clinician if further clinical guidance and support is required.
Refer client accordingly for future assistance
Call patients within 24 hours of missing their appointments and link them back to care.
Hand patients not found telephonically to WBOTS for home tracing.
Monitor outcome of home tracing and record accordingly.
Refer patient returned to care after defaulting, for enhanced adherence counselling.
Maintain confidentiality and ethical standards at all times
Exercise adherence to the relevant legislation, Codes of Conduct and SOPs
Identify own developmental needs and implement approved recommendations
Comply with all safety policies, practices and procedures according to prescribed legislation
Report all unsafe activities to Senior Management/Human Resources
Adhere to decision making procedures
SELECTION CRITERIA
**Education**
Grade 12 and /Post matric qualification - social science / equivalent diploma/degree
HTS Certificate
Basic Computer Skills
**Advantageous**
Index testing services
GCP certificate
Good communication skills
Advanced Adherence Counselling Certificate
Social Science Degree or related degree
**Experience**
At least 2 years’ HIV / TB testing and counselling in a Key Populations programme
At least one-year experience in providing support/counselling to HIV Infected Individuals dealing with psychosocial issues that influence Health-seeking behaviour
Knowledge in Index testing services
Knowledge on latest HIV care and treatment, PrEP guidelines
Adherence counselling
MSM / Trans gender recruitment
Experience in peer education
**Requirements**:
SA Citizen or valid work permit to work in South Africa
Basic computer skills
Willing to work overtime
Willing to travel within / outside of the local working area
Social networking strategy
Good team leadership skills
