Faircape Health Tokai Estate is a Sub Acute Care and Rehabilitation Centre which provides restorative care and rehabilitation for patients recovering from a variety of medical conditions that include Medical, Neurological, Orthopaedic, Surgical and Respite. At Tokai Healthcare, the focus is mainly on acute conditions that immediately affect patients' rehabilitation progress.

Our world-class care aims to bridge the gap between hospital and home. Our Multidisciplinary Team (MDT) of leading healthcare professionals administers care in the most therapeutic environment possible and supports healing outcomes in all aspects.

**Duties relevant to this role**:

- Facilitate subacute viewings
- On Day 2 following the MDT meeting, ensure that the patient rehabilitation contract is completed and understood by the patient.
- Confirm discharge dates of patients
- have they reached their goals and are ready for discharge
- Is there a max on medical aid affecting stay
- Do they need to be approached to ask if they want to pay privately
- Discharge report / discharge follow up card
- Ensure family/ patient is aware of MDT feedback
- Ensure discharge patient experience surveys are completed one day prior to discharge / discharge report / follow up appointment
- Book clients for and manage the post discharge clinic
- Complete the GAS jotform once the patient is discharged
- Make follow-up calls to patients who were unable to attend the post-discharge clinic.
- Ensure specialist referrals are completed and sent through to the referring specialist
- Follow up patients who is being transferred back to hospital

**Qualifications**
- Degree in Occupational Therapy or Physiotherapist with a BSc degree with more than 1 years clinical experience
- HPCSA registration
- Malpractice insurance

**Experience and Knowledge**
- Proven experience working in a similar role
- Experience working in an acute/sub-acute environment

**Skills and Attributes**
- Experience in a clinical environment and working with ill patients and family members
- A commitment to early intervention, a whole-of-person approach to rehabilitation and improved levels of functioning of the injured client;
- The ability to engage positively with clients acknowledging their individual circumstances;
- Good communication, organisation and liaison skills and ability to initiate activity where required;
- The ability to manage multiple tasks concurrently and maintain focus;
- The capacity to maintain and broaden networks of relevant professional service providers;
- Good time management
- Good Crisis management skills
- The ability to refer clients to service providers and if necessary seek out additional professional assistance or direction;
- Ability to perform basic screening at follow-up clinic to flag for referral to services required.
- Knowledge of the relevant legislation claims procedures, rehabilitation provisions and administrative requirements;
- Have an understanding of ICF Framework and SMART goal-setting skills

**Specific Requirements**:

- Vaccinated against covid-19 or prepared to be vaccinated prior to commencing job

**Hours**:
Weekdays 08h30-17h30 onsite

Salary on offer is from R27 000- R30 000 Cost to Company per month depending on experience and qualifications.
