**Job Advert Summary**:
**Purpose of the position**:
A Case Manager provides oversight and supervisory role to Case Officers toward the achievement of specific program key performance areas, with a focus on retention. This cadre will provide support to achieve optimum adherence and retention for HIV-diagnosed patients until they are eligible to be decanted. Additionally, this cadre will offer general administration support, data capturing, collation, and reporting as required.

**Minimum Requirements**:

- Matric
- Post-matric qualification (M&E, Basic HIV training, or experience in lieu of training)
- A minimum of 3-5 years’ relevant experience in the health sector, managing or implementing HIV/AIDS and STI (HAST) programmes with knowledge of HIV and TB treatment cascades
- At least 2 years’ experience in supervisory/managerial and mentoring or coaching role at facility level
- Relevant public health experience (hospital and primary health care level), the NGO environment will be advantageous
- Advanced computer literacy skill (e.g. MS Office package, SyNCH, Tier.Net, SharePoint, etc.)
- Basic knowledge or training in project management
- Valid driver’s licence
- Clear criminal record
- Able to communicate in English and isiXhosa

**Duties and Responsibilities**:

- Developing relevant tools and enforce use of recognised tools and registers
- Support documentation of activities on recognised source documents
- Identify and name key participants and their roles in the facility Case Management Model
- Supervision and mentoring
- Support Case Officers to document appointment dates on spreadsheets and allocate dates of return for all care milestones
- Coordination, monitoring & evaluation
- Establish processes/systems
- Outline the monitoring system, targets and standards for each Case Officer
- Report writing
- Stakeholder engagement at facility level
