**Job Advert Summary**:
Our client, a leader in the private healthcare industry, is urgently looking for an experienced **Clinical Care Coordinator **to work at their **medical emergency service contact centre **in **Rivonia** in** Johannesburg**.

This is a** **fixed-shifts** **opportunity, **starting ASAP**.

**Minimum Requirements**:

- Qualified Emergency Care Technician (ECT) / Emergency Care Practitioner (ECP) / Advanced Life Support Practitioner (ALS) / Professional Nurse (PN)
- Active registration with the relevant professional council
- Post-graduate degree in clinical or healthcare management will serve as an advantage
- At least 2 years’ prior experience working with patients in either an administrative, clinical or care coordination role
- Experience in effectively managing a care team involved in patients’ care
- Virtual care and/or home healthcare experience and chronic disease management experience will serve as an advantage
- Case Management experience will serve as an advantage
- **Exceptional communication and administration skills**:

- Working knowledge of the local and international healthcare market
- Familiar with relevant policies and procedures
- Proficient in Microsoft Office (Word, Excel, Outlook, etc.)
- Strong written and verbal skills in English
- Quality Management knowledge and experience
- Strives to identify and deliver on patients' needs and continuously focus on the patient experience
- Well-developed client service skills
- Virtual Healthcare and Telemedicine experience
- Medical Terminology experience
- Business Acumen and commercial awareness

**Duties and Responsibilities**:
The role entails liaising with facility clinical teams, medical aids, allied healthcare service providers, and approved service providers to 'enrol/admit' patients into the Hospital at Home programme and follow carefully created care pathways and adjust in line with the patients clinical condition.

Ensure that all patients who qualify for care coordination are adequately cared for through the entire patient journey. Be an advocate for patients and ensure that the care team effectively coordinates patient care from request to post-discharge. Responsible for monitoring and reviewing patients' biometrics and immediate well-being (via virtual link).
- Optimise the patient experience by systematising a care plan for several patients at once
- Effectively and efficiently manage the care team involved in patient care
- Manage patient engagement and discuss the care plan designed or prescribed by the care team
- Manage, coordinate and ensure clinical services are scheduled in a way that promotes the continuity of patient care
- Evaluate outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team
- Manage all financial processes and practices of the team to facilitate cost savings
- Reporting, analysis and feedback
- Review virtual patient well-being and ensure patient monitoring is maintained within required clinical parameters
- Personal effectiveness - Keep abreast of the latest technology and related developments within scope of position / Communicate and resolve issues and/ or opportunities in a professional manner / Ensure quality client service principles are maintained / Demonstrate behaviour congruent with company values
