Linkage Officer

**Job Advert Summary**:
**Purpose of the position**:
The Linkage officer will ensure inmates that are tested (HIV) and screened (TB / STI) are counselled and initiated on treatment. Integrated collaboration with external stakeholders will ensure inmates are linked to care on release. The post is roving and offers support to multiple correctional centres within a management area

**Minimum Requirements**:

- Grade 12
- ATTIC Certificates
- Experience with TIER.NET will be an advantage
- Minimum of 5 years’ experience working in the HIV, TB programme in South Africa
- Fluent in English / isiXhosa or any South African languages
- Clear criminal record

**Duties and Responsibilities**:

- Conducts HIV counselling and testing
- Promotes and screens inmates for TB / STI and VMMC
- Refers inmates living with HIV to a nurse clinician for appropriate ARV initiation.
- Refers at risk inmates to the nurse clinician for appropriate PrEP initiation
- Ensures clients are linked to appropriate TB/HIV and STI services within a community health facilities on release (linkage)
- Accurately records and reports using approved source data platforms (TIER.net), tools and templates
- Prepares reporting tools (CPP) and summary sheets for the review by data monitor and nurse clinician
- Ensures all source documentation, including client files, registers, and project reports, are complete and accurate
