1. Provide high-quality, evidence-based Physiotherapy assessment and intervention to a diverse group of clients including residents of aged care and private clients across both clinic-based and community settings; under the any of the following schemes
a. Medicare Better access to mental health care scheme
b. SIRA Workcover
c. NDIS
d. DVA
e. Victims of crime
f. Private fee-paying clients
g. Residents of aged care facilities

2. Understand and select appropriate assessment/s and interventions based on client needs and goals and write and support these decisions in writing

3. Participate in and support the philosophy of person-centred practice by engaging in collaborative, holistic goal setting that reflects the individual’s needs and priorities

4. Ensure ongoing monitoring of the consumer’s needs and progress, and collaboratively review goals as required to provide through strength-based and capacity building approaches

5. Develop rapport with clients to build a relationship of trust and mutual respect

6. Set and maintain a high standard of clinical practice

7. Liaise with medical practitioners, referrers and other interested parties and provide written progress reports as per the policies of Concentric Healthcare Services. Advise on and discuss the management of patients with musculoskeletal problems when consulted by physiotherapists from other areas, or other medical personnel

8. Participate in and design and run exercise classes catering for falls prevention, core stability, arthritis, etc.
9. Assess and prescribe mobility aids and appliances to meet a clinical needs

10. Treat patients within designated times (30-45mins assessment including first treatment; 20-30 mins follow up appointments)

11. Undertake any other duties that We reasonably specify as falling within the parameters of Your position
**Pain Management and Functional Physiotherapy in Aged Care Facilities**:
1. Pain management services which include:
a. Establishing and maintaining the pain management programme
b. Implementing the pain management programme as per the allied health directive or otherwise as directed
c. Providing weekly listing of each resident’s compliance status with the pain management programme
d. Completing pain management related ACFI interventions for at least 2 weeks prior to ACFI submission (note: Residents are to continue on the pain management program unless clinically indicated to remove a Resident(s) from the program or the Resident(s) consistently refuses pain treatment)
e. Reviewing the effectiveness of the interventions at quarterly intervals and modify as clinically necessary after the ACFI claim has been submitted and on an ongoing basis
f. Daily documenting of the pain management program to ensure compliance
g. Weekly meetings with facility staff, such as the Clinical Manager, to discuss changes to residents’ status, function and compliance with the pain management programme
h. Maintain equipment and supplies relevant to the pain management programme

2. Functional physiotherapy services which include:
a. Completing comprehensive physiotherapy assessments including transfer/mobility, falls risk and pain management
b. Contributing to resident care plans including appropriate allied health directives into the pain management programme as per relevant legislation and ACFI rules
c. Re-assess physiotherapy assessments where required due to declining health or frailty, return from hospital, following a fall, and any other risk factor that could impact Resident's condition
d. Participate in regular falls prevention programme meetings
e. Assess and prescribe mobility aids and appliances to meet a clinical need
f. Facilitate group exercise programs
g. Provide regular informal training opportunities with the physiotherapy aide
h. Create exercise and rehabilitation programs for relevant staff to complete
i. Provider manual handling orientation for new staff, students or staff forwarded by the facility
j. Provide falls prevention programme orientation for new staff, students or staff forwarded by the facility

3. Work cooperatively with facility staff and physiotherapy aides

4. Liaise and work closely with facility staff to ensure the resident’s needs are met

5. Report any incidents to both Facility management and Concentric management teams

6. Comply with Workplace Health and Safety (WHS) and Australian Aged Care Quality Agency (AACQA) policies and procedures

7. Undertake any other duties that We reasonably specify as falling within the parameters of Your position

1. Liaise with families, other health professionals and service providers as required to ensure person-centred, holistic and quality service delivery.
