Job Description
We are looking for a skilled and detail-oriented Clinical Associate to support our growing Chronic Care Management (CCM) program. In this role, you will work closely with physicians, nurses, and other healthcare professionals to coordinate and optimize care for patients with chronic conditions such as diabetes, hypertension, heart disease, COPD, and more. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for improving patient outcomes. Key Responsibilities
- Conduct comprehensive assessments of patients' health status, including reviewing medical histories, medications, and treatment plans.
- Collaborate with physicians and care teams to develop and implement personalized care plans for chronic patients.
- Educate patients on managing their conditions, adhering to treatment protocols, and making lifestyle changes to improve their health.
- Monitor patient progress through regular follow-ups, telehealth consultations, and remote monitoring tools.
- Document patient interactions and updates in electronic health records (EHR) systems accurately and efficiently.
- Identify gaps in care and proactively address barriers to adherence, ensuring timely interventions.
- Serve as a trusted point of contact for patients, providing guidance, answering questions, and addressing concerns.
- Stay up-to-date with the latest developments in chronic disease management and evidence-based practices.
Qualifications
- Bachelor's degree in Nursing, Health Sciences, or a related field; additional certifications (e.g., RN, LPN, CCM) are a plus.
- 2+ years of experience in clinical settings, preferably in chronic care management, primary care, or case management.
- Strong understanding of chronic diseases, treatment protocols, and best practices in patient care.
- Proficiency in using EHR systems and telehealth platforms.
- Exceptional interpersonal and communication skills, with the ability to build rapport with patients and colleagues.
- Ability to multitask, prioritize effectively, and work independently in a fast-paced environment.
