- Collaborate with patients, healthcare providers, care teams, facilities, and schemes throughout the patient journey
- Advocate for patients and ensure effective coordination of care from request through post-discharge
- Monitor and review patient biometrics and immediate wellbeing
- Maintain continuous records across multiple electronic platforms
- Optimise patient experience by coordinating multiple care plans simultaneously
- Provide reporting, analysis, and feedback
- Provide tele-health support and review virtual patient wellbeing
- Ensure patient monitoring is maintained within required clinical parameters
Requirements
- Degree / Diploma leading to qualification as a Professional or General Nurse
- Diploma / Degree in relevant healthcare field
- Qualified Paramedic, Emergency Care Practitioner, or Professional Nurse
- At least 3 years' prior experience working with patients in an administrative, clinical, or care coordination role
- Organisational skills
- Attention to detail
- Knowledge of relevant policies and procedures
- Working knowledge of the local and international healthcare market
- Computer literacy (MS Office)
- Excellent communication, feedback, and follow-up skills
- English written and verbal skills
