You will: Perform comprehensive psychosocial assessments and craft individualised care plans. Provide counselling, crisis intervention and ongoing emotional support to patients and families. Facilitate family meetings, discharge planning and smooth community reintegration. Advocate for resources, benefits and funding to optimise long-term outcomes. Collaborate daily with the interdisciplinary to deliver seamless, evidence-based care. Maintain accurate, confidential documentation compliant with SACSSP standards and contribute to quality-improvement initiatives.
