About the Role
Our client is seeking an experienced and compassionate Social Work Case Manager to join their dedicated team in Klerksdorp . This essential role involves providing comprehensive support and case management services to individuals and families facing social challenges. You will be responsible for assessing client needs, developing intervention plans, and connecting them with vital community resources. The ideal candidate is committed to client advocacy and possess strong skills in counseling, resource navigation, and case documentation. Key Responsibilities
- Conduct thorough assessments of clients' social, economic, and emotional needs.
- Develop and implement individualized case plans, setting clear goals and objectives.
- Provide direct counseling and supportive interventions to clients.
- Facilitate referrals to external agencies and services, such as housing, employment, and healthcare providers.
- Monitor client progress, document interventions, and adjust case plans as necessary.
- Collaborate with families, community members, and other professionals to ensure comprehensive support.
Requirements
- Bachelor's degree in Social Work or a relevant field.
- Registered with the South African Council for Social Service Professions (SACSSP).
- Minimum of 4 years of experience in case management or social work practice.
- Demonstrated ability in client assessment, intervention planning, and counseling.
- Knowledge of local social services and resources within the Klerksdorp area.
- Excellent report writing, communication, and interpersonal skills.
