Key Responsibilities
- Lead and manage the coordination of remote social care services, ensuring effective and efficient delivery.
- Develop and implement comprehensive care plans, leveraging digital tools for client assessment and progress tracking.
- Provide remote guidance, support, and advocacy to clients facing various social challenges.
- Collaborate with community partners and external agencies via virtual communication channels to secure resources and services.
- Mentor and supervise junior social care staff, offering virtual training and support.
- Maintain meticulous and confidential digital records of client interactions, assessments, and outcomes.
- Stay abreast of current social welfare policies and best practices relevant to remote service delivery.
