Responsibilities

• Assisting in addressing the psychosocial needs of patients and families through in-depth assessments based on assigned duties.
• Evaluating patients for transition planning requirements, emotional support, and community resource needs.
• Educating, coordinating, and collaborating with various healthcare professionals to devise comprehensive discharge plans.
• Ensuring optimal resource utilization and appropriate length of stay.
• Participating in the patient's treatment plan and attending daily Multidisciplinary rounding.

Qualifications

• LMSW, LCSW, or LMHC certification mandatory.
• Preferably certified in New York State PRI assessment.
• Proficiency in written and verbal English communication.
• Knowledge of Spanish or other dominant community languages is advantageous.
• Establishing professional relationships with physicians, patients, families, peers, and supervisors.
• Demonstrated sensitivity, compassion, flexibility, and creativity.
• Ability to thrive under pressure, exercise sound judgment, and demonstrate leadership.
